A gift form that names the fund

Staff publish one form for one appeal. The amount, the fund, and the person who paid stay together, so a pantry drive does not land in the same bucket as a building gift.

What the form holds onto

One appeal, one form

A food drive, a scholarship, and a year-end ask each get their own form. Staff can see which appeal the money was for without splitting a general total by hand.

A few amounts, or their own

Offer the amounts you hope people will choose, and leave a field for a different number. The donor is not forced into a preset.

Processing cover is a choice

The form can ask the donor to cover card processing. That line appears only when they opt in. The gift itself is still recorded as the gift.

A confirmation they can keep

The note they receive names the fund and the date. Year-end statements start from that record instead of a downloaded spreadsheet.

Questions about this page

Can one form collect for two funds?
Give each appeal its own form. A combined form makes the designation harder to explain later, so staff keep one purpose on each page.
Does the donor have to cover processing?
No. Covering the card cost is optional. If they skip it, the gift is still recorded and the processor fee is separate from the gift amount.
Where does the thank-you live?
On the person who gave, with the fund and the date. The next staff member can find that gift without opening the payment inbox.
